Memory Care Partnership
A Nurse Geriatric Care Manager understands the effects of progressive dementia and the changes the client experiences throughout the course of the disease. The Care Manager is able to interpret observed effects and communicate for the family and facility staff.
As part of our initial assessment and ongoing evaluation, the nurse care manager will administer Mental Status Questionnaires to the client to assess cognitive status and changes. Appropriate interventions will be implemented in the plan of care and discussed with the family, facility and physician.
Ongoing evaluation of our clients allows us to make necessary changes in the plan of care resulting in improved quality of life. For those with cognitive impairment, our goal is to increase safety, decrease falls, manage anxiety and behavioral issues, and support the facility staff by providing professional nurse consultation and partnership in care.
Medication management
A Professional Registered Nurse Care Manager provides daily, weekly, or bi-weekly medication management, monitoring and clinical evaluation for residents living in an assisted living environment. This specialized program includes RN oversight, administration of injectable therapies including insulin and heparin therapies, diabetes management, prescription refills/orders, physician communication, medication side effect monitoring and management, and patient/family education regarding prescribed therapies.
